Getting the right care for your loved one early in his or her end-of-life journey often delays the need for long-term care in an institutional setting. However, whether your loved one is living at home, at an assisted living facility or in a nursing home, you can relax in the knowledge that his or her care is being coordinated by experts who have extensive knowledge about the costs, quality, and availability of resources in the community.

Your Elder Care Coordinator serves as your support line, and your advocate. Your Elder Care Coordinator will:
